MediTelecare is the nation’s leading telemedicine provider of behavioral health care to skilled nursing, assisted living and independent living facilities seeks, an experienced full time Pharmacist with clinical, long-term care consultant, behavioral health and/or informatics experience. This specialized position is responsible for analyzing and reporting on MediTelecare’s quality improvement initiatives with its skilled nursing partners

Responsibilities

ESSENTIAL FUNCTIONS:

·Conduct and prepare MediTelecare “MediReview” assessments involving patient evaluation and analysis via electronic health record (EHR) use.

·Prepare, analyze and report on facility data in order to optimize care

·Formulate quality medication recommendations that adhere to CMS regulations and MediTelecare policies and procedures

·Participate in the development of clinical guidance in order to reduce polypharmacy, inappropriate medication use, antipsychotic or anti-anxiety/hypnotic use in the skilled nursing setting

·Aid in drug information, formulary development and development of quarterly facility presentations in order to optimize care

·Build relationships with physicians, nurse practitioners and other prescribers in order to optimize patient medication selection and care

·Aid compliance department in clinician clinical audit reviews to assure quality documentation and clinical practice that adheres to MediTelecare policies and procedures

·Ability to perform MTM and/or chart review activities effectively as needed


Skills & Qualifications

KNOWLEDGE, SKILLS AND ABILITIES:

  • Must be      a self-starter
  • High      level of organizational skills and proven follow up ability
  • Knowledge      of CMS skilled nursing regulations
  • Thorough      understanding of healthcare technologies including but, not limited      to:  using Electronic Health Record      Systems, Microsoft Outlook, Microsoft Excel, Microsoft Powerpoint, Adobe      PDF and internet/cloud based applications

·Skill in assessing and prioritizing multiple tasks, projects and demands

  • Must be      able to effectively and professionally communicate with facility staff,      management and colleagues

                                                             

EDUCATION, LICENSING, EXPERIENCE:


· Doctor of Pharmacy degree (PharmD) from an accredited college or university in Pharmacy

·Current, unrestricted Pharmacist license in the state of practice

·Previous experience in the long-term care consulting, behavioral health or informatics a plus

·High level of organizational skills with exceptional attention to detail

·Valid driver’s license in good standing
