Care Coordinator Responsibilities

  • Responding to potential patient inquiries for tele-behavioral health services, screening calls with patients and work closely with providers
  • Coordinating care with primary care offices, assisted living settings, and other healthcare practices utilized by our patients
  • Supporting intake processes including helping customers with registering their accounts, become a “concierge service” for patients.
  • Assisting with scheduling appointments
  • Making reminder calls prior to visits
  • Problem-solving technical issues that arise during sessions
  • Administrative charting duties (notating no-shows, documenting calls from customers, uploading forms and medical records)
  • Clinical intake/triage
  • Guiding referrals for additional services
  • Fielding emergency calls after hours
  • Responsible for BHI- Behavioral Health Integration- data entry.  Will be assigned a set amount of patients per month.  Involves obtaining patient information from facility’s EHR system and entering into a form in MediTelecare’s EHR system

Care Coordinator Qualifications

  • LNA, CNA, MA or Pharma Techs.  This is a great job for you!
  • Healthcare experience, familiarity with medical records. 
  • Able to communicate with patients effectively and professionally, families, facility staff and colleagues
  • Excellent phone and computer skills
  • Customer service driven
  • Knowledge of computer and computer programs including Electronic Health records
  • Knowledge of medical and behavioral health terminology
  • Scheduling
  • Intake
